To participate in middle school athletics, the following items must be completed and on file with the athletic office:
1. Review the MSHSL Eligibility Brochure. Click one of the following links to complete ALL pages (including the last page). We suggest completing the forms with the Noteability App on your students iPad. Upload the completed pages to the on-line registration.
2. Complete the Online Registration Form
3. Upload Sports Physical to the registration form. | (completed by health care provider) valid for three years.
4. Pay Sports Fee - Fees can be paid online at the rSchoolToday e-Store with credit card. You can pay for multiple sports by choosing up to one sport from each season and adding it to your cart. Athletic fees can also be paid by check at the main office. Please make checks payable to Highland Park Middle School.
